___ College of Nursing and Health Sciences Graduate Admission Application

• Did you complete the essay in the application?

• Did you sign and date the application?

• Did you make a copy for your records?

• Did you send it to FIU College of Nursing and Health Sciences, University Park Campus, Graduate Nursing Program, HLS 2 Room 485, 11200 SW 8th St., Miami, FL 33199?

___ Transcript Request for each prior school attended to be sent to:

• 1) Florida International University; and

• 2) College of Nursing and Health Sciences.  NOTE:  The transcript sent to the CNHS must be a "paper" transcript.

NOTE: Send the applications before you complete the transcript request(s).

___ Copy of current State of Florida RN license

___ Three (3) letters of recommendation

___ Official copies of GRE score sent to:
