The objectives of the NURSING PROCESS
HEALTH PROMOTION ASSIGNMENT are to:
Develop a nursing process care
plan that focuses on the health promotion of an
adult within a
family.
Utilize internet and course resources to develop an assessment, nursing diagnoses, planning (with specific outcome criteria, interventions, and evaluation) for the nursing process health promotion care plan.
Your NURSING PROCESS HEALTH PROMOTION ASSIGNMENT is the following:
Select an adult
client. You can use
yourself as the client for the health promotion paper. If you prefer not to provide information on yourself, you can
use a friend or relative or other client.
Write the nursing process paper IN THE THIRD PERSON (“he,”
“she”) so the reader will not be able to identify whether it was
completed by you or another individual.
ASSESSMENT
Perform a health promotion assessment on an individual. Each type of
assessment instrument has a unique focus; however, there will be some overlapping in the
dimensions that are assessed. Assessment data
should consist of the following:
1.
General
lifestyle appraisal:
General
lifestyle instruments assess a variety of dimensions to provide an
overall
“picture” of the individual’s lifestyle. There are many web sites that offer a free assessment of
general lifestyle.

summarize the findings (strengths and weaknesses) for EACH dimension.

Health risk appraisal:
A health risk appraisal assesses an individual’s risks to his/her health, such as cancer or

connected to health risks, you may find similarities between lifestyle

Fitness plays a significant role in an individual’s health

Nutritional
appraisal.
Nutrition also plays a significant role in an individual’s health and health risks. There

Injury prevention

Psychosocial
appraisal.
Research has shown that psychosocial dimensions, such as how an
individual perceives
self (his/her degree of self-esteem), and
interpersonal relationships and communication influence health. Go to the

Addiction
appraisal.
Research has also shown that addictions impact health.
The most common addictions in the
U.S. are to nicotine and
alcohol. These addictions and others (i.e., eating disorders, substance abuse, gambling
addiction, sexual addiction, and even internet addiction) can be
devastating to an individual's (and his/her family's) health. There are self-test instruments that assess for an addiction. Go to the web site that is listed below, review

Perceived
degree of control over health status. Individuals
must perceive some degree of control over their
health to make changes.
If individuals feel there is nothing they can do to change their health risks, then health
promotion
education will be ineffectual. Go to the web

PLANNING/OUTCOME
CRITERIA
As
noted in the text, if the client concludes that he/she already has positive functioning in a certain
area,
the client can use this information to help him/her reach a higher
level of functioning.
If
the client is in need of positive functioning in a certain pattern
area(s), the client can
Collaborate with the
nurse in setting priority
goals (be certain to indicate a time frame) for increased positive functioning;
Collaborate
with the nurse to identify possible behavior changes, assign priorities to
behavior changes; make commitment to change behavior (self-contract);
identify effective reinforcements and rewards; determine barriers to
change;
Collaborate
with the nurse in exploring available resources to assist with the change, and
develop a schedule (be specific) for implementing the behavior.
Make the
plan/outcome criteria client-centered, e.g., the "client
will...." Relate the
plan/overall goal to the nursing diagnosis.
For example, if the WELL
client has "low
weight,"
then the overall goal
may be to have "appropriate
nutritional
intake." Use as
evidenced by as introduction to stating measurable
(time/amount) outcome criteria:
NURSING INTERVENTIONS
Self-responsibility
is emphasized for implementing the plan.
The client is the primary decision maker, and
the
nurse's role is to ascertain that the assessment is accurate and
complete, and the goals are
individualized and attainable.
Make the
interventions
nurse-centered.
Be specific (so anyone
could carry out the interventions) and state frequency/amount/time, e.g., the "nurse will...."
Suggested
nursing strategies (be specific as to how often, how much,
etc.) for behavior changes
include:
Supporting;
Teaching;
Consulting;
Coordinating;
Facilitating;
Counseling;
Enhancing
Intervention references should be researched-or factually based.
For
example, if a health promotion goal is to stop smoking, references should include quality internet resources or
journal articles that relate successful strategies to stop smoking.
EVALUATION
Evaluation
is ongoing and a collaborative effort between client and nurse. Evaluation is based on the plan/outcome
criteria (both
short and long term). Look at your
measurable
plan/outcome criteria and state whether the client has achieved the
criterion. e.g.,
